Technology Request Form

There’s now an easier way for 4J employees to request new apps, software and hardware and consult with the Technology Department on new initiatives using technology.  This has been approved by the Superintendent and Executive Leadership Team in an effort to safeguard our district’s digital environment and support innovative, effective teaching and learning.

Employees must complete and submit the new online form outlining their needs to technology staff. Use the form to request new apps or software, new hardware or support the early planning stages of a project, among other things. Each request will be routed to the appropriate technology staff member for review and responsive follow-up.

Why is this process required?

This process allows us to:

  • Keep the network operating at maximum capacity for all users
  • Proactively manage growing cybersecurity threats
  • Ensure compliance with student data privacy laws (COPPA, FERPA, etc.)
  • Align technology purchases with educational goals and student safety
  • Protect our network, systems, and sensitive information
  • Ensure safety for student and staff 
  • Stretch our budget during lean times and use taxpayer funds and grant dollars responsibly and transparently

Use this short form to request:

  • New apps or software (including Chrome extensions and Google add-ons)
  • New technology-related hardware
  • Orders of 15 or more of the same item from our approved technology list, or purchases over $750
  • Project support or thought partnership during early planning stages
  • Other support or resources from the Technology Department

Upon submission, your request will be routed to the appropriate Technology Team Member for review and expedient follow-up.

You are required to consult the Technology Department:

  • When connecting any device to the 4J network
  • When purchasing a subscription to a platform that requires your 4J username to login
  • Before installing software or apps on a 4J-owned device
  • If integrating with any existing 4J system or platform
  • For support, training, purchasing or rostering subscriptions
  • For questions regarding data privacy, security, or terms and conditions
  • When using district funds, grant funds, or personal dollars for any of the above

Please note: This form is not for reporting repair or break/fix issues. Those requests should continue to be submitted through the Helpdesk ticket system.
